Internal hernias rarely occur upon gastrectomy but may happen several months after surgery. We present a rare case of internal hernia developing shortly after laparoscopic gastrectomy for gastric cancer. A 76-year-old man complained of epigastric pain and nausea but no other symptoms on the sixth day after surgery. Symptoms aggravated, and fever developed on postoperative day 7. Computed tomography (CT) showed intestinal herniation through Petersen¡¯s space. Laparoscopic examination of the abdominal cavity enabled us to confirm the existence of an internal hernia beneath the afferent loop, which was managed by manual reduction using minilaparotomy. The patient recovered well, and a post-operative CT scan showed no herniation through the gastrojejunostomy defect.
